Good Faith Estimate

You have the right to understand the expected cost of your care.

Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use insurance to pay for their care may be entitled to receive a Good Faith Estimate of the expected cost of scheduled health care services.

A Good Faith Estimate is not a bill. It is an estimate of reasonably expected charges based on the information available when the estimate is prepared.

Who can receive a Good Faith Estimate?

You may receive a Good Faith Estimate if you are uninsured or if you have health insurance but do not plan to submit a claim or use your insurance benefits for the services being estimated.

You may also request a Good Faith Estimate before scheduling services.

When will I receive it?

Federal timing requirements depend on when services are scheduled or when an estimate is requested.

  • If you request a Good Faith Estimate before scheduling services, it generally must be provided within 3 business days of the request.
  • If services are scheduled 3–9 business days in advance, the estimate generally must be provided within 1 business day after scheduling.
  • If services are scheduled at least 10 business days in advance, the estimate generally must be provided within 3 business days after scheduling.

A Good Faith Estimate is generally not required under this federal provision when a service is scheduled fewer than 3 business days in advance.

What will the estimate include?

The Good Faith Estimate will describe reasonably expected charges for the services included in the estimate based on the information known at the time.

Because counseling needs can change, the frequency, duration, or type of services ultimately provided may differ from the initial estimate. When appropriate, an updated estimate may be provided.

How Flourish provides Good Faith Estimates

Flourish Mental Health Counseling provides Good Faith Estimate information to applicable clients as part of the intake process and upon request.

Good Faith Estimates and other clinical intake documents may be provided through the practice’s designated client system.

If your bill is substantially higher than the estimate

If the billed charges from a provider or facility are at least $400 more than the Good Faith Estimate from that provider or facility, you may be eligible to use the federal patient-provider dispute resolution process.

The federal dispute process has deadlines. Information about eligibility, current procedures, and required timeframes is available through the Centers for Medicare & Medicaid Services.
